Oregon POLST Registry Now Accessible Nationwide Through MyDirectives’ A|D Vault Exchange via Carequality and eHealth Exchange

DALLAS & PORTLAND, Ore.--(BUSINESS WIRE)--Dec 2, 2025--

HIT Commons, the Oregon POLST Registry, and MyDirectives today announced a major interoperability milestone. For the first time, validated Oregon POLST forms are available nationwide through Carequality and eHealth Exchange, and can be retrieved directly within participating EHR workflows.

This achievement was made possible through collaborations with St. Charles Health System, the first in Oregon to activate POLST retrieval using Epic’s Care Everywhere and MyDirectives’ HITRUST-certified A|D Vault Exchange.

POLST forms are portable medical orders that document a patient’s treatment preferences during medical emergencies, such as resuscitation instructions, when they may not be able to communicate their wishes. Oregon is one of the few states with a mandated-reporting POLST system, and its registry serves as the authoritative source for the most current, validated version of each form.

The effort builds on MyDirectives’ long-standing role as a Responder Organization on the eHealth Exchange. The network supports secure, nationwide data exchange among thousands of healthcare organizations and manages more than a million weekly queries to MyDirectives’ A|D Vault Exchange.

The Oregon initiative is one of the first in the nation to demonstrate how a state-level POLST registry can integrate seamlessly with national interoperability frameworks, providing a scalable model for other states seeking to modernize access to POLST and advance care planning documents.
