Healthcare-Shift.com offers parents, teens and young adults resources to help young adults make the shift from pediatric to adult healthcare, including a healthcare checklist, conversation guide and appointment guide, as well as video with experts that parents and teens can watch together.

Through Healthcare-Shift.com, the company will provide education to build a better understanding of what the move to adult care means for teens, along with resources for parents and kids to start building the skills needed to take charge of their healthcare beginning earlier in childhood.

“In the U.S., when a person turns 18, they’re considered a legal adult, and there are many changes that happen in the healthcare system related to that. 4 The trouble is, we don’t prepare parents and kids for that transition in the way that we anticipate the shift from high school to college or work,” says Susan Shanske, Clinical Social Worker and Director, Transitional Care Support, BRIDGES Adult Transition Program at Boston Children’s Hospital. “Yet that shift from pediatric to adult care can be just as disruptive and can result in young adults not getting the healthcare they need.”

The handoff from pediatric to adult healthcare, the complexity of the U.S. health insurance system and the challenges of chronic conditions combined represent a serious risk for teens and young adults to manage. 5 According to a 2018 national poll of 1,200 randomly selected adults, 45 percent of 18- to 29-year-olds do not have a primary care physician 1, who is often the front line for access to most types of healthcare in the U.S. For patients with chronic conditions, the consequences can be significant.

Through “watch together” videos, appointment guides and a healthcare shift checklist of essential information, teens and young adults can develop a plan that works
  • Caregivers: Teaching children the skills needed to advocate for their own health and wellness is just as important as learning to read, drive or use a bank account. Videos, information and a conversation guide for caregivers offers easy-to-implement ideas for getting underway
